Student Summer Camp Waiver & Parental Consent Agreement By selecting “I Agree” below, I confirm that I am the parent or legal guardian of the student being registered and that I have read, understand, and agree to the following: Participation & Assumption of Risk I give permission for my child to participate in the Student Summer Camp and related activities organized by Grandview Church. I understand that participation may include travel to and from camp, group activities, outdoor recreation, worship gatherings, and other camp-related events. I acknowledge that participation involves inherent risks, including but not limited to transportation, physical activity, environmental conditions, and interaction with other participants. I voluntarily assume all such risks on behalf of my child. Release of Liability To the fullest extent permitted by law, I release, waive, and forever discharge Grandview Church and its affiliated partners and sponsors, including their directors, officers, employees, volunteers, and agents (the “Released Parties”), from any and all claims, liabilities, or demands arising out of or related to my child’s participation in the Student Summer Camp. This includes, but is not limited to, claims for personal injury, illness, death, property damage, or loss, whether caused by ordinary negligence or otherwise, to the extent permitted by law. Indemnification I agree to indemnify and hold harmless the Released Parties from any claims or causes of action brought by or on behalf of my child arising out of participation in camp activities, except to the extent prohibited by law. Transportation Authorization I give permission for my child to be transported to, from, and during camp activities by Grandview Church staff, volunteers, or designated drivers using church-owned or privately owned vehicles. Medical Authorization In the event of illness or injury, I authorize Grandview Church staff or volunteers to obtain emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for all medical expenses incurred and that the church does not provide medical insurance for participants. Behavior Expectations I understand that students are expected to follow camp rules and demonstrate respectful behavior. Serious misconduct may result in dismissal from camp at my expense, and I agree to cooperate with church leadership if necessary. Photo & Video Use I grant permission for Grandview Church to use photographs and/or video recordings of my child taken during camp activities for church and ministry purposes, unless otherwise indicated on a separate form. Agreement By clicking “I Agree,” I acknowledge that this electronic agreement is binding and equivalent to a signed written agreement, and that I voluntarily consent to all terms listed above.
